Treatment of Thigh Pain / Meralgia paresthetica

Meralgia paresthetica is a pain diorder, characterized by burning pain, tingling and numbness n outer part of your thigh.

It is caused by compresion of the lateral femoral nerve, which carries sensation to upper leg. it is a nerve entrapment syndrome.

What are the causes ??

wearing tight clothing, obesity , fibroid in uterus, or sudden weight gain and pregnancy.

it may be due to fluid accumulation in the abdomen causing increased abdominal pressure, Local trauma to pelvic bone, disease such as diabetes.

Why it happen?

In most patients, this nerve passes from your groin to upper thigh without any interruption. but if this lateral femoral cutaneous nerve becomes trapped under this inguinal ligament as above reason, then patient feels pain in outer and upper part of thigh.

Aggravating factors: postures, walking, prolonged standing or sitting

Relieving factors: lying down and rest.

Have you had recent surgeries?

Have you had recent injuries to your hip area, such as from a seat belt in a motor vehicle accident?

Do you regularly do repetitive activities that affect your hip area, such as cycling?

Preventive measure for meralgia paresthetica.

Avoid wearing tight clothing.

Maintain a healthy weight, or lose weight if you’re overweight.

Diagnosis:

  • It ias mostly diagnosed by clinical history and physical examinations. doctor will do strength testing and reflex testing to exclude other causes.
  • Imaging studies; mri or ct imaging of your hip and pelvic area might be needed to exclude other pathologies.
  • Electromyography: to rule out muscle and nerve disorders.
  • Nerve conducton study: by stimulating the nerve, we can diagnose nerve pathology. also comparision of lateral cutaneous nerve on each side may be performed.
  • Diagnostic nerve blockage by ultrasound machine it is very accurate for diagnosis.

Treatment:

  • Pharmacotherapy

we can give pregabalin, gabapentin, or amitryptiline , OR NSAIDS .but these work well in few patients and long lasting pain relief is not observed in some patients.
