Treatment of Chest Wall Pain

chest pain, whether on the right or the left side, can happen for many reasons, such as problems in the digestive system, problems in the lungs, and even problems in the heart.

In all cases, pain should be assessed and treated as needed.

Alarming sign: If your pain occurs frequently, is very intense, worsens with time, or if it comes with other symptoms such as tingling in the arm and face, difficulty breathing, or fainting, call an ambulance or proceed to a hospital immediately.

These symptoms can be a sign of a life-threatening problem.

Mostly pain at right sided of chest is temporary.

Stress and Anxiety:

Excessive stress and anxiety can create panic attack, it may induce similar heart attack like symptoms, like sudden chest pain. This panic attack disappears within few minutes. Usually pain is perceived in the middle part of the chest, but it may radiate to the right side of the chest wall.

What to do: The best way to alleviate discomfort caused by a panic attack is to try to calm down by regulating your breathing relaxing any tense muscles

Muscle strain:

Trauma or overuse of muscles , while during activities, like sports, or more exercise in gym. This pain does not happens instantly, it may appears after few days. Other symptoms of strained chest muscles include increased pain with palpation, slight swelling, and difficulty moving your arms.

What to do: Applying ice to the affected region for 15 to 20 minutes, three to four times a day, will help with pain relief.

Also, you can do a light massage over the affected region with an anti-inflammatory cream along with medications.

Acid reflux (or GERD):

Acid reflux, also known as heartburn, is a common condition that occurs , when stomach acid travels up the esophagus, causing a burning sensation (particularly after eating). This discomfort can sometimes be painful, and can be felt in the general chest area or on the right side.Gastroesophageal reflux disease (GERD) is generally accompanied by other adjuvant symptoms like frequent need to burp, a bitter taste in the mouth, dry cough and feeling like there’s a lump in the throat.

What to do: Depending on the severity, reflux symptoms can be relieved by simple diet changes, such as avoiding large meals and avoiding fatty and spicy foods.

However, in some cases, pantoprazole like medication can be given to block acid reflux. If the discomfort does not improve with diet changes, a consult with a gastroenterologist is recommended to start adequate treatment.

Costochondritis : It refers to inflammation of the cartilage that joints  sternum to rib

  • The most common sites of pain are the anterior cartilage segments of the 2th to 5th ribs at the front of the chest near where the ribs join the breastbone.
  • Typically pain is felt in the middle of the chest, but it can radiate to the right or left side.
  • Aggravating factors: This pain increases with pressure changes in the chest like, with activity, intense coughing, moving or twisting or when taking deep breaths. Conversely, it decreases as movement stops or with quiet breathing.

Causes:

The exact cause is unknown but may be associated with unusual or excessive physical activity, minor trauma particularly repetitive and/or excessive over of use of arms ere also appears to be a relationship between chest wall deformities a

How to diagnose?

  • The reproducible tenderness but without swelling you feel when you press on the sternal and rib joints (costosternal and costochondral junctions respectively) is a constant feature of costochondritis.

Characteristic of Pain.

  • It may be ,aching ,stabbing ,sharp Burning ,tearing pain that worsens when you move your chest, twist your torso, or raise your arms
  • pain that increases when you breathe deeply, sneeze, or cough
  • it may also come and go and vary in duration from hours to days. It may be accompanied by numbness, tingling, shooting pain that extends to your back or neck.

Differential diagnosis
